Required Camp Forms & Documents

Please download and carefully review each document on this page. Some forms require signatures and must be completed before your student arrives at Camp Be.Love.

CampBe.Love. Student Registration Form

Every student attending Camp Be.Love. must have a completed Student Registration Form on file. Please complete all sections and submit the form according to the directions provided.

CampBe.Love. Adult Registration Form

All adult leaders, volunteers, and chaperones attending Camp Be.Love. must complete the Adult Registration Form. Please fill out all required information and submit the form before camp.

Emergency Action Plan (EAP)

The Emergency Action Plan is a required State of Texas document for every camper attending camp. Each parent or guardian should read the entire EAP document and complete the EAP Child/Adult Acknowledgement Form confirming they have reviewed it.

2026 Immunization Records Form

All campers must provide a copy of their current immunization records. Please attach a copy of your student's shot records with the required camp paperwork. If your student is exempt from immunization requirements, a parent or guardian must complete and sign the exemption form included in this section.

Lake Tomahawk Rules & Policies

Please review the Lake Tomahawk rules, including the bathing suit policy and other camp guidelines. One exception to the Lake Tomahawk rules is that Camp Be.Love. will allow students to bring cell phones during camp. We ask all students to use them responsibly and respectfully throughout the week.

The Camper Checklist will help you prepare for camp and ensure your student arrives with everything needed for a safe, fun, and meaningful week.

Medication Form

If your student will be bringing medication to camp, please download and complete the Medication Form. Place the completed form, along with all medications, inside a sealed Ziplock bag and bring it to registration on opening day.
